By design
Two people, every document
A document leaves the workspace only after someone at your firm approves it, and that person is never the one who generated it. So a workspace needs at least two people: one to prepare, one to review. Who reviews is your firm's decision.

Review before release
In development
A person at your firm approves a document before it is released; a changed answer produces a new, unapproved document, and a withdrawn approval is recorded with its reason. Done from the firm workspace.

Who approved what, and when, is written to an append-only, hash-chained ledger and anchored off-site every hour.
